Reversal Strategies for DOAC-Related Bleeding: Promoting Optimal Use of Reversal Agents

With the emergence of specific reversal strategies for direct-acting oral anticoagulants (DOACs), it is critical for health-system pharmacists to take a leading role in ensuring that these agents are used appropriately. In this educational activity, the faculty review evidence-based protocols and guidelines for the use of DOAC reversal agents and provide strategies for establishing a standardized, evidence-based approach for managing the use of DOAC reversal agents within a health system. Approaches to ensure appropriate use of anticoagulation reversal agents for DOAC-related bleeding are illustrated using case scenarios.

Learning Objectives

  • Identify evidence-based protocols and guidelines related to the use of reversal agents for direct-acting oral anticoagulants (DOACS) that standardize clinical practice and address National Patient Safety Goals.
  • Identify approaches for ensuring cost-effective use of anticoagulation reversal agents for DOAC-related bleeding.
  • Develop a plan for establishing a standardized, evidence-based interprofessional approach for managing the use of DOAC reversal agents within the health system.
